Exciting employment opportunity with the ability to make a positive impact in the community: This posting is for one temporary 24-hour week, contract Social Worker to provide Case Management services to High-Risk pregnant women of Alamance County. The CMHRP position provides 100% CMHRP OB Care Management/CMHRP services for pregnant non-eligible Medicaid recipients who are determined to be at risk for poor birth outcomes. This service includes assessment, planning, facilitation, care coordination, evaluation, and patient advocacy to address health care and preventative service needs of pregnant and postpartum women. Care Coordination focuses on coordination of the patients' medical, nutritional, educational, social, and emotional needs with private and community resources to promote healthy pregnancy outcomes. Community visits and patient home visits are best practice for meeting program measures. This position will also be required to staff emergency shelters as mandated by the State Public Health Office; assist with Public Health outbreak investigations and response, and perform additional duties as required by the Nursing Supervisor, Nursing Director, Medical Director, and Health Director.
