About the position

The Transition Coordinator role is focused on facilitating the implementation of a life skills curriculum for youth transitioning back into the community. This position involves working closely with various stakeholders, including educational institutions and state agencies, to ensure that students receive the necessary support and resources for successful reintegration. The role requires participation in administrative committees and follow-up consultations to monitor and assist students' progress post-release.

Responsibilities

  • Facilitate the implementation of a life skills curriculum, titled Next Steps.
  • Utilize the Transition Coordinator Handbook to standardize transition practices.
  • Evaluate the five 'Transition Consultations' to be completed by every student under the guidance of an assigned teacher.
  • Participate weekly in Administrative Release Committees (ARC) to confirm and reinforce students' reentry plans.
  • Conduct follow-up phone calls to released youth at 1, 3, 6, 9, and 12 months post-release.
  • Represent the Indiana Department of Correction- Division of Youth Services at conferences and meetings.
  • Assist in developing partnerships with Indiana's Department of Workforce Development, HIRE, and Indiana University's HOPE Mentoring.
  • Help develop individualized Education Learning Plans for each youth upon entry at the facility.
  • Work with state, county, and private agencies to provide re-entry services for youth transitioning back into the community.
  • Assist students in obtaining social security cards, state identification, driver's licenses, birth certificates, and additional documentation.
  • Support students in transitioning back to public schools and provide information on employment, apprenticeship programs, and military service.

Requirements

  • Valid Driver's License is required.
  • Experience working with youth in a transitional or educational setting.
  • Strong communication and interpersonal skills.
  • Ability to collaborate with various stakeholders, including state and private agencies.

Nice-to-haves

  • Experience in curriculum development or implementation.
  • Knowledge of re-entry services and resources for youth.
  • Familiarity with the Indiana Department of Correction policies.
