About the position
GrapeTree Medical Staffing LTC is looking for a dedicated LPN / LVN to provide long-term care in a travel position based in Mount Horeb, Wisconsin. This role involves working 40 hours per week on evening shifts for a duration of 13 weeks, contributing to the healthcare community by delivering high-quality patient care in various settings.
Responsibilities
- Provide nursing care to patients in long-term care facilities.
- Administer medications and treatments as prescribed by physicians.
- Monitor patient health and report any changes to the healthcare team.
- Assist with daily living activities and ensure patient comfort.
- Maintain accurate patient records and documentation.
Requirements
- Valid LPN / LVN license in the state of Wisconsin.
- Experience in long-term care settings preferred.
- Ability to work evening shifts and adapt to changing schedules.
- Strong communication and interpersonal skills.
Nice-to-haves
- Experience with electronic health records (EHR).
- BLS certification.
