About the position

The CxC Virtual Care Nurse at AdventHealth is responsible for coordinating care, educating clients, and providing protocol-driven interventions to improve clinical and whole health outcomes. This role involves virtual assessments, collaboration with healthcare providers, and the development of care plans, all while ensuring exceptional customer service and adherence to established workflows.

Responsibilities

• Coordinate care and educate clients regarding their health needs.
• Provide virtual assessments of healthcare needs and develop care plans.
• Collaborate with healthcare providers to ensure comprehensive care delivery.
• Document all interactions and maintain accurate consumer records.
• Utilize critical thinking skills to triage symptomatic consumers and provide appropriate interventions.
• Provide referrals to healthcare providers and community resources as needed.
• Maintain knowledge of AdventHealth services and workflows to direct inquiries effectively.
• Adhere to established workflows, scripting, and department protocols.

Requirements

• Effective communication skills in English, both orally and in writing.
• Ability to type at a minimum speed of 45 words per minute.
• Proficiency in Microsoft Suite programs and Adobe.
• Technological aptitude for mastering CRM, telephony, and electronic health records systems.
• Associate of Science degree in Nursing from an accredited school.
• Minimum of 2 years of clinical experience as a Registered Nurse (RN).
• Multistate licensure as a Registered Nurse (RN).
