About the position

Pennsburg Manor is seeking a dedicated Wound Care Coordinator to join our nursing center team. This position is crucial in leading and integrating a comprehensive Skin Health and Wound Management program, ensuring that our patients receive the highest quality of care. The Wound Care Coordinator will work closely with nursing center management to develop and implement strategies that promote skin health and prevent pressure injuries. This role requires a compassionate and motivated Registered Nurse (RN) who is committed to patient-centered care and has a strong focus on wound management. As a Wound Care Coordinator, you will oversee the clinical aspects of skin and wound care, serving as the clinical expert for skin, wound, and incontinent products. You will also manage durable medical equipment related to wound prevention and care. This position is not only about providing care but also about leading a team, fostering collaboration, and ensuring that all team members are aligned with the goals of the skin health program. You will be responsible for monitoring outcomes and implementing best practices in wound care, making a significant impact on the overall health of our patients. The work schedule is primarily Monday to Friday during the day, but occasional evening or weekend hours may be required. Responsibilities

  • Lead and integrate a fully developed Skin Health and Wound Management program in collaboration with nursing center management.
  • Support central supply as the clinical expert for skin, wound, and incontinent products.
  • Oversee durable medical equipment related to the prevention and management of wounds.
  • Monitor outcomes and implement best practices in wound care.
  • Foster collaboration among the nursing center team to ensure alignment with skin health goals.

Requirements

  • Must possess a valid RN license (MPT/DPT with leadership review/approval).
  • Completion of Skin Health Team Lead orientation training and Wound Basics within 90 days of hire is required.
  • Commitment to attend monthly support calls is required.
  • Must have a nationally accredited wound certification (CWS, CWCN, WCC, CWON, WOC, AWCC) or be committed to complete Advanced Wound Training and national accredited Wound Certification within 1 year of hire.
  • Knowledge and experience with clinical charting, incident reporting, and investigation response is required.
  • Strong knowledge base of acceptable standards of care for nurses, providers, therapists, dietitians, and others on the nursing center team is required.
  • Experience in team building and demonstrating respect for the interprofessional team in wound management is required.
  • Knowledge of national wound guidelines and wound products is required.
  • Knowledge of nursing home regulations and survey process is required.
  • Strong skills in Microsoft Word, Excel, Database Management, and PowerPoint are required.

Nice-to-haves

  • Experience in a nursing center or long-term care environment.
  • Previous leadership experience in a clinical setting.
  • Familiarity with electronic health records (EHR) systems.
