About the position

We are seeking compassionate and motivated Registered Nurses (RNs) with a focus on wound care to join our team as a Wound Care Coordinator. In this role, you will lead and integrate a fully developed Skin Health and Wound Management program in collaboration with nursing center management. Your expertise will be crucial in supporting central supply as the clinical expert for skin, wound, and incontinent products, as well as overseeing durable medical equipment related to the prevention and management of wounds. This position is essential in ensuring that our patients receive the highest quality of care, as skin health is a vital indicator of overall health. As a Wound Care Coordinator, you will be responsible for developing and implementing strategies to achieve nursing center skin health and pressure injury prevention goals. You will also be involved in training and mentoring other nursing staff on best practices for wound care, ensuring that all team members are equipped with the knowledge and skills necessary to provide exceptional care. Your role will require a commitment to ongoing education and professional development, as you will be expected to complete Skin Health Team Lead orientation training and Wound Basics within 90 days of hire. Additionally, you will need to maintain a national accredited wound certification or commit to completing Advanced Wound Training and national accredited Wound Certification within one year of hire. Responsibilities

  • Lead and integrate a fully developed Skin Health and Wound Management program in collaboration with nursing center management.
  • Support central supply as the clinical expert for skin, wound, and incontinent products.
  • Oversee durable medical equipment related to the prevention and management of wounds.
  • Train and mentor nursing staff on best practices for wound care.
  • Develop and implement strategies to achieve nursing center skin health and pressure injury prevention goals.

Requirements

  • Must possess a RN license (MPT/DPT with leadership review/approval).
  • Must complete Skin Health Team Lead orientation training and Wound Basics within 90 days of hire.
  • Commitment to attend monthly support calls is required.
  • Must have national accredited wound certification (CWS, CWCN, WCC, CWON, WOC, AWCC) or be committed to complete Advanced Wound Training and national accredited Wound Certification within 1 year of hire.
  • Knowledge and experience with clinical charting, incident reporting, and investigation response is required.
  • Must have a strong knowledge base of acceptable standards of care for nurses, providers, therapists, dietitians, and others on the nursing center team.
  • Experience in team building and demonstrating respect for the interprofessional team in wound management is required.
  • Must have knowledge of national wound guidelines and wound products.
  • Knowledge of nursing home regulations and survey process is required.
  • Strong Microsoft Word, Excel, Database Management, and PowerPoint skills are required.

Nice-to-haves

  • Experience in a nursing center or long-term care environment.
  • Familiarity with electronic health record systems.
