About The Position

The Youth and Family Case Manager at Bluebonnet Trails Community Services is responsible for delivering case management, skills training, and medication education to youth and their families in community-based settings. This role emphasizes evidence-based treatment and person-centered recovery planning, assisting families in accessing community resources and providing rehabilitative services and crisis intervention as needed. The position requires a passion for working with youth with severe emotional needs and involves collaboration with a multidisciplinary recovery team.

Requirements

• Bachelor's degree in a social, behavioral, or human service field or equivalent coursework.
• Knowledge of mental illness and case management functions.
• Valid Texas Driver's License and maintain approved driver status throughout employment.

Nice To Haves

• One year of experience in mental health assessment and/or crisis intervention and screening.
• Master's degree in psychology, social work, or a related human service field.
• Bilingual skills.

Responsibilities

• Builds and maintains therapeutic relationships with individuals through engagement.
• Provides psychosocial rehabilitation and links individuals with community support.
• Performs assessments to identify strengths and needs of individuals.
• Completes Person Centered Recovery Plans with goals and objectives for individuals.
• Documents services in a timely manner.
