About the position

The Practice Facilitator (PF) at The Fund for Public Health in New York City (FPHNYC) is responsible for leading quality improvement initiatives in primary care settings, focusing on chronic illness management, particularly blood pressure control, cholesterol management, and diabetes prevention. The PF will work closely with healthcare providers to enhance patient health outcomes through technical assistance, workflow development, and evidence-based programming. This role is integral to fostering partnerships between healthcare practices and community health initiatives, ultimately aiming to improve health equity and social justice in New York City.

Responsibilities

  • Manage a caseload of 30-40 practices engaged in quality improvement initiatives using practice facilitation strategies.
  • Conduct on-site or remote visits to provide technical assistance to practices improving patient health outcomes, particularly for hypertension, pre-diabetes, diabetes, smoking, and other cardiovascular-related priority health areas.
  • Support workflow redesign and EHR documentation to improve office efficiency for care management.
  • Coach on preventive-health features of EHRs and how to use them as part of QI activities.
  • Serve as a liaison between primary care practices and self-management programs for chronic illness.
  • Act as a content expert for evidence-based intervention (EBI) referral workflow development in small practices.
  • Participate in forums discussing performance feedback and sharing best practices for QI efforts and practice transformation programs.
  • Serve as a liaison between practice staff and various BeHS teams to provide feedback on QI efforts.
  • Deliver presentations and trainings to internal and external audiences.

Requirements

  • Two plus years of relevant work experience including involvement in quality improvement programs, Patient Centered Medical Home (PCMH), and Quality Payment Program (QPP) initiatives at health care practices.
  • Ability to travel within the 5 boroughs of NYC.
  • Advanced skills in data management as well as the ability to analyze and present data to groups and individuals.

Nice-to-haves

  • Bachelor's degree; Master's degree a plus.
  • Two plus (2+) years of project management experience.
  • Experience completing or leading quality improvement initiatives in a clinical setting.
  • Knowledge of the Patient Centered Medical Home (PCMH) and Quality Payment Program (QPP) initiatives.
  • Outpatient primary care experience and familiarity with hypertension, diabetes, smoking and other cardiovascular-related priority health areas.
  • Ability to work collaboratively with physicians, staff, and external organizations.
  • Strong computer skills, Electronic health record and training experience.
  • Excellent time management and communication skills.
