About the position
The position involves providing case management services as part of a multidisciplinary team within the Eliot Program for Assertive Community Treatment (PACT). The role requires developing treatment plans, conducting assessments, and providing clinical interventions to clients with severe and persistent mental illness. The candidate will be responsible for managing a caseload of 5-8 clients and will need to perform home visits, necessitating reliable transportation and a valid Massachusetts license.
Responsibilities
- Provide case management services as part of a multidisciplinary team
- Develop treatment plans with consumers
- Act as a primary case manager to consumers in accordance with Eliot's policies and PACT standards
- Participate in completing assessments as assigned by the Team Leader or Lead Clinician in a timely manner
- Assess client's functions, strengths and needs
- Develop therapeutic connections with each client and provide clinical interventions
- Provide problem-solving supports and interventions to clients
- Manage a caseload of 5-8 clients
Requirements
- Bachelor's degree in Psychology, Social Work, or a related field preferred
- Valid driver's license required
- 1+ years prior experience working with adults having severe and persistent mental illness required
- MUST have a reliable vehicle for home visits
- MUST have a Massachusetts license
