About the position

The RN Case Manager at Consistent Care is responsible for managing care for patients with complex medical and social needs. This part-time role involves assessing, developing, implementing, coordinating, monitoring, and evaluating care plans to optimize patient health. The position requires both in-person and telephonic interactions with patients, ensuring that care is delivered where the patients are located, such as their homes or clinics. The role emphasizes a passion for helping others and creative problem-solving skills to support vulnerable individuals in the community.

Responsibilities

  • Conduct assessments to identify individual needs and develop specific care plans.
  • Facilitate patient access to services appropriate to their health needs.
  • Assess behavioral health needs and coordinate services as needed.
  • Make home visits for patient assessment or education.
  • Monitor and evaluate the effectiveness of care management plans and modify as necessary.
  • Interface with community partners to advocate for patients' needs.
  • Work closely with community health workers to address social determinants of health.
  • Manage and delegate daily tasks to assigned staff.
  • Facilitate community-wide meetings to encourage collaboration among agencies.
  • Establish and build working relationships with community resources.
  • Communicate frequently by telephone, averaging 20 calls a day.
  • Contribute to bi-weekly virtual staff meetings.
  • Occasionally transport patients to appointments.

Requirements

  • Current RN license in WA state.
  • Basic computer skills and Electronic Medical Record experience.
  • Valid WA driver's license and car insurance.
  • Excellent oral and written communication skills.
  • Ability to work positively in stressful situations.

Nice-to-haves

  • Medical and behavioral assessment experience.
  • Motivational interviewing experience.
  • Bachelor of Science in Nursing (BSN).
  • Certified Case Manager (CCM).
  • Dual diagnosis experience with mental health and substance abuse.
  • Home health or case management experience.
