About the position
The Hospice Registered Nurse (RN) Case Manager is responsible for providing comprehensive nursing care to patients in a hospice setting. This role involves assessing patient needs, developing care plans, and coordinating with interdisciplinary teams to ensure quality end-of-life care. The RN Case Manager will also provide education and support to patients and their families, ensuring comfort and dignity during the hospice process.
Responsibilities
- Conduct patient assessments to determine care needs.
- Develop and implement individualized care plans for hospice patients.
- Coordinate with interdisciplinary teams to provide comprehensive care.
- Educate patients and families about hospice services and care options.
- Monitor patient progress and adjust care plans as necessary.
- Document patient care and maintain accurate medical records.
- Provide emotional support to patients and families during end-of-life care.
Requirements
- Current RN license in North Carolina.
- Experience in hospice or palliative care preferred.
- Strong assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a team environment.
Nice-to-haves
- Certification in hospice and palliative nursing (CHPN).
- Bilingual skills are a plus.
