About the position

IDR is seeking a Case Management Assistant to join one of our top clients in San Francisco, California. This role is pivotal in facilitating and implementing the transition of care and discharge plans for ambulatory patients. The Case Management Assistant will collaborate closely with case managers, social workers, patients, and families to ensure timely and efficient discharges. This position requires maintaining accurate, current, and legible documentation in the electronic medical record, capturing key discussions and updates on the status of referrals and discharge plans. Additionally, the role involves providing necessary clerical support, including copying, faxing, scanning, and data entry. The Case Management Assistant will work closely with the care coordination team, participating in daily huddles and departmental meetings. Proactively identifying and resolving barriers to the transition of care plans is essential, ensuring effective communication and problem-solving. Building positive relationships with customers and partners is crucial, demonstrating respect and effective communication skills. The assistant must comply with confidentiality policies, HIPAA regulations, and department standards when handling patient information. Responsibilities

Requirements

  • High School Diploma or equivalent education/experience.
  • A minimum of 1-year recent relevant experience in a similar role.
  • Strong oral and written communication skills, capable of interacting effectively with diverse groups.
  • Ability to work effectively in a fast-paced environment with shifting priorities and competing demands.
  • Demonstrates strong time management and organizational skills, with the ability to prioritize tasks and work independently.
  • Intermediate PC skills and word processing skills.
  • Experience with electronic medical records and other healthcare-related software systems.
