About the position

The Acquired Brain Injury/Moving Forward Plan Program Coordinator II will oversee and facilitate the development of new residential programs for individuals transitioning from nursing facilities to community residences. This role involves collaboration with various stakeholders, including the ABI Director and residential providers, to ensure successful program implementation and individual placements. The coordinator will also manage the siting process for new homes and track the progress of individuals' transitions into the community, ensuring that all necessary plans and referrals are completed in a timely manner.

Responsibilities

  • Coordinate and monitor regional 24/7 new program development efforts for individuals eligible for ABI-RH and MFP RS Waivers.
  • Liaison between the Regional ABI Director and providers to track progress on new program development efforts.
  • Identify barriers to residential site openings and track provider referrals related to individual placements.
  • Manage the sharing of group profiles of individuals with potential residential providers for appropriate placements.
  • Ensure that all Transitional Assistance plans are submitted completely and on time to MassAbility.
  • Work with the team to track priorities such as meaningful day supports and community engagement opportunities after individuals are placed.

Requirements

  • Bachelor's degree in Business Administration or related field.
  • At least 1 year of experience in business management or program coordination.
  • Mid-level administrative experience.
  • Strong organizational skills and conflict management abilities.
  • Proficiency in Microsoft Word and Excel.

Nice-to-haves

  • Experience working with individuals with disabilities or acquired brain injuries.
  • Ability to communicate effectively with multiple organizations and individuals at varying levels of responsibility.
  • Experience in human service provider organizations.
  • Cultural competence and sensitivity in working with families from diverse backgrounds.
