About The Position

The ACT Case Manager position at Spurwink Services involves providing community-based case management for adults with serious mental illness. The role requires collaboration with an interdisciplinary team to develop and implement Individual Service Plans (ISP) and ensure clients have access to necessary resources and support.

Requirements

• MHRT-C certification or MHRT-C eligible, or a Certified Rehabilitation Counselor or Employment Specialist, or a Certified or Licensed Alcohol and Drug Counselor.
• Bachelor's degree from an accredited four (4) year institution of higher learning preferred.

Responsibilities

• Provide intake/assessment when clients are referred, and ongoing assessment as required.
• Facilitate Individual Service Plan team meetings and develop a plan of care (ISP) for the adult with input from the team.
• Oversee implementation of ISP, monitor and document progress.
• Maintain regular contact with team members and other community professionals involved with client.
• Participate in a team-based approach to care, collaborating with various healthcare providers.
• Identify and assess physical and behavioral health related risk factors and ensure clients and families are aware of community services and resources.
• Advocate for clients and families.
• Provide support through phone contact and face-to-face meetings, including home visits and community outreach.
• Partner with clients' primary care, medical, and other providers.
• Participate in individual and group supervision, required training, and case consultations.
