We are a thriving and growing organization, and our team is expanding across sites to support this growth. We are currently hiring an Administrative Aide - Home Health/TOC based in Worcester. The Administrative Aide's responsibility is to provide clinical and administrative support to the Home Health Utilization Nurse, the Transition of Care Nurse and the agencies Kennedy Community Health partners with. These responsibilities are primarily clerical and include scanning, filing, preparing questionnaires, answering phone calls, faxing and any other relevant responsibilities as assigned. Responsibilities

  • Download, sort and store home health related faxed documents in a frequency determined by the Home Health Utilization program needs.
  • Return completed paperwork to appropriate Kennedy Community Health provider for review and signature.
  • Follow up with provider as needed.
  • Send signed and completed documents to appropriate requesting agency.
  • Answer phone calls from agencies, providers and patients.
  • Utilize an internal excel workbook to track patients and their services.
  • Escalate concerns to Home Health Utilization Nurse appropriately.
  • Perform general clerical duties, including document preparation, copying and filing.
  • Operate a variety of office machines, including computers, copier, fax machine, postage equipment, and other office machinery.
  • Track and book annual physical exams.
  • Review Admission, Discharge and Transfer reports and collaborate with RN.
  • Develop relationships with participating hospitals and community partner organizations.
  • Perform utilization reviews and coordinate identification of potential high dollar/utilizations cases and identify/recommend opportunities for cost saving and improving of care.
  • Participate in performance improvement projects and processes.
  • Send letters to patients who have been unable to reach.

Requirements

  • Intermediate level excel proficiency
  • Proven record of customer service
  • Experience with home health agencies or ambulatory care agencies
  • Proven ability to work independently
  • Work experience in a culturally diverse workplace
