About the position

The Administrative Assistant for Care Coordination/Case Management at Care Crafter Health is responsible for providing essential administrative support to the care coordination team. This role involves assisting with client intake, scheduling appointments, maintaining client records, and facilitating communication among clients, caregivers, and healthcare providers. The ideal candidate will be detail-oriented and organized, ensuring efficient operations within the department.

Responsibilities

  • Assist with client intake and scheduling appointments
  • Maintain accurate and up-to-date client records
  • Coordinate communication between clients, caregivers, and healthcare providers
  • Assist with data entry and documentation tasks
  • Assist with coordinating care plans and ensuring all necessary services are in place

Requirements

  • High school diploma or equivalent required
  • Prior experience in a healthcare or human services setting preferred
