About the position

The LPN/RN Admission Care Liaison role at Ohio's Hospice is a full-time position focused on providing compassionate care to patients at the end of their life journey. The position involves assessing patient needs, presenting hospice services, and developing patient-centered care plans in collaboration with an interdisciplinary team. The role requires effective communication with healthcare providers to ensure timely admissions and coordinated care for patients.

Responsibilities

  • Assess and evaluate the patient and family needs, including physical, spiritual, and psychosocial aspects.
  • Present hospice philosophy and services to referred patients and families to facilitate timely admissions.
  • Develop and implement initial patient-centered care plans with interdisciplinary team members.
  • Communicate patient information to the Hospice Physician for certification of terminal illness.
  • Assist with patient assessments for facility-based contracted beds and manage symptoms as needed.
  • Collaborate with interdisciplinary team members and facilitate patient transfers to home or other facilities.
  • Establish coordinated care plans with attending physicians and hospital staff for new and existing patients.
  • Participate in Continuous Quality Improvement activities to enhance service quality.
  • Precept new staff to foster professional relationships and support their orientation.

Requirements

  • Registered LPN or RN with the State of Ohio without any board actions.
  • Two years of nursing experience preferred.
  • CPR Certified.
  • Sufficient computer skills for documentation of services and care.
  • Ability to drive during daytime, nighttime, or inclement weather.
  • Valid Driver's License with a safe driving record.
  • State minimum automobile insurance coverage.
