About the position

The Admissions and Marketing Coordinator at Heartland Country Village is responsible for promoting the organization's services to potential residents, families, and referral sources. This role involves generating new admissions, coordinating with referral sources and insurance companies, and conducting educational programs and community outreach activities. The position supports the mission of the community by ensuring a smooth transition of care for individuals needing skilled nursing services.

Responsibilities

  • Promote the services of the organization to potential residents and referral sources.
  • Cultivate and enhance relationships with hospitals, nursing homes, and other healthcare providers.
  • Maintain regular contact with referral sources to meet patient and family needs.
  • Follow up on referrals to educate potential admissions and families about facility services.
  • Conduct on-site assessments of referrals to determine care needs.
  • Facilitate the transition of new admissions, ensuring thorough communication and continuity of care.
  • Assist in obtaining necessary records for new admissions.
  • Participate in community functions to promote awareness of the facility.
  • Contribute to the development of the facility's marketing plan.
  • Prepare reports on agency activity for the Administrator.
  • Maintain accurate tracking of referrals and territory management system.
  • Stay informed about the community's healthcare market and governmental policies affecting agency goals.
  • Complete intake information with new admissions as needed.
  • Negotiate with third-party payers for service authorizations.
  • Communicate effectively with referral sources, facility staff, patients, and families.

Requirements

  • Excellent communication skills.
  • Knowledge of the local market for skilled nursing services and familiarity with case managers and discharge planners.
  • Ability to work independently and exercise discretion and judgment.
  • Friendly and approachable demeanor.
  • Strong time management and organizational skills.
  • Minimum of 2 years of experience in healthcare.
  • Bachelor's degree in Marketing, Business, or related field, or an LPN or RN with 2 years of experience in a skilled nursing setting.
