About the position

The Admissions Coordinator: Care Management at PeaceHealth is responsible for managing the admissions, discharges, and transfers of patients within the swing bed program. This role involves pre-screening referrals, coordinating with various stakeholders, and ensuring timely documentation in the electronic medical records system. The coordinator plays a crucial role in supporting the Care Management Team and facilitating discharge arrangements for patients.

Responsibilities

  • Coordinate admissions, discharges, and transfers while ensuring patient privacy and confidentiality.
  • Assess patient referrals to confirm they meet admission criteria.
  • Assist in arranging home health skilled services and durable medical equipment.
  • Coordinate post-discharge follow-up appointments and arrange new primary care providers for un-doctored patients.
  • Document medical necessity and obtain treatment authorization related to swing/SNF benefits.
  • Issue Medicare notices and confirm patient demographic information.
  • Confirm outpatient therapy or post-hospital appointment arrangements.
  • Document discharge planning interventions in CareConnect promptly.
  • Screen visitors and telephone calls to determine urgency and facilitate appropriate referrals.
  • Perform clerical duties including scheduling assistance and daily assignment management.

Requirements

  • Preferred: Associate Degree
  • Preferred: PT Assistant, Medical Assistant or LPN
  • Required: Skilled nursing facility or swing bed background and/or previous health insurance experience.
  • Preferred: Minimum of 3 years administrative experience in a professional medical office environment.
  • Preferred: Knowledge of insurance industry (third party payers, state and federal insurance payers).
  • Preferred: Experience with payer portals and marketing sales background.

Nice-to-haves

  • Experience with statistical compilation and analysis of databases related to quality assurance and program effectiveness.
