About the position
The Admissions Coordinator at Pinnacle Hospice is a full-time role focused on coordinating patient referrals for hospice services. This position requires a dedicated and experienced Licensed Practical Nurse (LPN) who will evaluate patient referrals, identify insurance coverage, and collaborate with healthcare professionals to ensure a smooth admission process for patients in need of hospice care.
Responsibilities
- Evaluates provided information of patients referred for hospice services.
- Identifies insurance coverage of hospice services.
- Collaborates with the hospitals' case management to facilitate safe, early discharge.
- Consults with physicians, nurses, social workers, discharge planners, and other disciplines to establish a coordinated home plan of care.
- Ensures that all admission forms are complete, clear, and within scope of service.
- Communicates with field staff and other hospice departments regarding status of referrals.
- Performs other duties as requested/assigned.
Requirements
- Graduate of an accredited school of nursing.
- Current license to practice as a licensed practical nurse in Louisiana (unencumbered).
- Minimum of two years experience, at least one of which is in the area of public health, home care, or hospice nursing preferred.
