About the position

The Admissions Coordinator at River's Edge Rehabilitation & Living Center plays a crucial role in managing the admissions process for patients in a skilled nursing facility. This position involves establishing relationships with local clinics and hospitals, ensuring a smooth admissions experience, and maintaining accurate records of available beds and resident information. The coordinator will work closely with various departments to promote a positive environment for both patients and staff.

Responsibilities

• Establish and maintain positive relationships with clinics and hospitals in the area.
• Respond to customer requests and concerns to promote a positive admissions and referral process.
• Supervise and review the collection of admission documents, ensuring completion and forwarding to appropriate departments.
• Conduct interviews of residents/guardians to obtain required information and signatures.
• Assist in the orientation program for residents and personnel.
• Maintain an accurate record of available beds.
• Develop and maintain a good working rapport with inter-department personnel and other departments within the facility.
• Maintain a resident waiting list in accordance with established policies and procedures.
• Keep current on Medicare/Medicaid provider regulations for skilled nursing facilities.
• Maintain daily and monthly census reports as directed.

Requirements

• Clinical background in healthcare.
• Community liaison experience in a healthcare environment.
• Knowledge of the community preferred.
• Strong communication and interpersonal skills.
• Admissions experience in a healthcare facility required.
• Ability to work well both independently and as part of an interdisciplinary team.
• Positive attitude and driven by a desire to serve others.
