About the position

The Admissions Director/Nurse Liaison at Fallbrook Rehabilitation and Care Center is a dynamic role focused on leading the admissions process with passion and precision. This position is responsible for managing the entire admission journey, ensuring a seamless experience for residents and their families while collaborating with various departments to optimize the admissions process. The ideal candidate will possess strong communication skills and a commitment to delivering compassionate healthcare.

Responsibilities

  • Lead the admissions process from initial inquiry to admission to the facility, ensuring a smooth transition for residents and families.
  • Establish, build, and maintain connections with referral sources, including via community outings.
  • Assess referrals for clinical capabilities and appropriateness of admission, bridging the clinical and non-clinical team for residents' needs.
  • Evaluate and recommend adjustments to admission practices to optimize efficiency and effectiveness.
  • Identify and pursue opportunities with new referral partners and sources, developing sales strategies to increase admissions.
  • Collaborate with the Director of Nursing, Administrator, and other department heads as needed to facilitate admissions.
  • Generate and analyze daily, weekly, and monthly census reports to track occupancy and identify trends.
  • Promote and enforce safe work practices, safety rules, and accident prevention procedures within the admissions process.

Requirements

  • Proficiency with Microsoft Office Suite.
  • Minimum of 1 year of experience in a nursing home or acute setting.
  • Knowledgeable about nursing and medical practices and procedures, as well as laws, regulations, and guidelines related to long-term care.
  • State of practice RN or LPN license is required.

Nice-to-haves

  • Bachelor's degree in Business is preferred.
  • Experience in a nursing home or acute care setting is preferred.
