About the position
The Admissions Director at Liberty Village of Clinton is responsible for coordinating the admissions process in a skilled nursing facility. This role involves developing relationships with various departments and community agencies to ensure that social service programs meet the needs of residents. The Admissions Director will also provide support and information to residents and their families regarding financial assistance programs and participate in interdisciplinary care planning.
Responsibilities
- Develop and maintain a good working rapport with intra-department personnel and community health agencies.
- Complete tours, admission processes, and discharge planning.
- Develop and implement social care plans and conduct patient/resident assessments.
- Refer patient/resident families to appropriate social services agencies.
- Provide information to families regarding Medicare/Medicaid and other financial assistance programs.
- Complete social histories for residents.
- Participate as a member of the interdisciplinary team in preparing care plans.
- Document conditions, attitudes, and responses of residents quarterly or as needed.
- Serve as a liaison between the facility, residents, families, and outside agencies.
- Provide social and psychological support for residents.
Requirements
- Experience in coordinating admissions processes in a healthcare setting.
- Strong interpersonal and communication skills to work with residents, families, and community agencies.
- Ability to develop and implement care plans and assessments.
Nice-to-haves
