About the position

The Admissions Liaison at our family-owned rehabilitation center plays a crucial role in ensuring a smooth admission process for patients. This position involves gathering essential information, conducting pre-admission assessments, and coordinating with various stakeholders to meet the needs of incoming residents. The Admissions Liaison will work closely with patients, their families, and healthcare providers to facilitate a seamless transition into our facility, ensuring that all necessary documentation and authorizations are in place for optimal care.

Responsibilities

  • Gather necessary information and complete pre-admission review assessments prior to patient admission.
  • Work with patients and their responsible parties to understand their expectations and functional levels before admission.
  • Complete payer verification inquiries and obtain necessary authorizations from Managed Care, Medicare, and Medicaid organizations.
  • Review current orders and specialty equipment needs for admission, coordinating with staff to ensure readiness.
  • Communicate necessary information to management and care teams upon admission confirmation.
  • Ensure the Admissions Contract is signed and uploaded to the resident's record electronically.
  • Develop and submit a weekly marketing plan to drive census expectations.
  • Complete daily referral logs to track and trend business effectively.
  • Build relationships with referral sources such as Case Managers, MDs, and community organizations to enhance referral pipelines.
  • Complete all training requirements in a timely manner.
  • Demonstrate superior customer service through professionalism and positive communication.
