About the position

The Admissions/MDS Nurse at Carespring Health Care Management is responsible for assessing new and existing patients to ensure their care needs are documented and addressed according to the MDS process. This role involves collaboration with other MDS nurses and interdisciplinary teams to improve resident outcomes and ensure compliance with regulations.

Responsibilities

  • Overseeing the MDS process following the RAI manual.
  • Working with other MDS nurses to complete initial assessments for new admissions.
  • Properly managing patient admissions scheduling, MDS assessment & care planning.
  • Completing required submissions weekly or as directed by the DON.
  • Reviewing validation report to ensure error-free submission.
  • Obtaining & reviewing QM's.
  • Working with IDT to improve resident outcomes.
  • Attending IDT Medicare & Medicaid meetings in collaboration with IDT to complete the MDS process.
  • Addressing the needs of residents by developing & updating comprehensive & nurse aide care plans.
  • Ensuring non-nursing sections of MDS & CAA's accurately reflect resident needs.
  • Monitoring care plans to identify resident needs.
  • Reviewing order recap reports & nurses' notes to ensure care plans are followed & determine whether additional OBRA MDS assessments are needed.
  • Monitoring residents' change of status & determining need for change per RAI.
  • Ensuring accuracy of supportive documentation according to MDS/RAI regulations.
  • Working with facility consultants to implement recommended changes.
  • Completing AIMS, Fall Risk scale, Braden, Progress note, Care Plan, nurse aide care plan (Kardex) paperwork.

Requirements

  • A current LPN or RN license in the state of Ohio.
  • A flexible work schedule & the ability to work independently.
  • An understanding of the MDS process & a willingness to continually learn.
  • Knowledge in PDPM & RUG.
