About The Position

Requirements

• Current Hawaii RN License in good standing.
• BSN preferred.
• Two years' experience in hospice, long-term care, or related field preferred.
• Strong communication and customer service skills.
• Ability to prioritize, problem solve, and manage multiple tasks independently.
• Ability to collaborate with the interdisciplinary team.
• Knowledge of psycho-social needs of patients.
• Familiarity with Medicare, Medicaid, and private insurance providers is a plus.
• Knowledge of Centers for Medicare and Medicaid Services guidelines related to hospice care preferred.

Responsibilities

• Respond to referrals from hospitals and community partners.
• Educate patients and caregivers about hospice services and the transfer process.
• Obtain and communicate relevant patient data for insurance benefits and care planning.
• Coordinate patient admissions and facilitate transitions from hospitals or contracted facilities.
• Document all activities in accordance with hospice protocol and regulatory requirements.
• Provide reports to the interdisciplinary team to ensure comprehensive patient care.
• Engage in educational activities to enhance professional practice.
• Perform other duties as required.
