About the position

The Admissions Nurse (RN) is responsible for planning, organizing, and directing hospice care, focusing on community health education and individual patient needs. This role involves comprehensive patient assessments, developing care plans, administering treatments, and coordinating with healthcare teams to ensure quality care for patients and their families.

Responsibilities

  • Complete an initial, comprehensive assessment of patient and family to determine hospice needs.
  • Provide a complete physical assessment and history of current and previous illness.
  • Utilize all elements of the nursing process to provide professional nursing care.
  • Assess and evaluate patient status by writing and initiating a plan of care.
  • Regularly re-evaluate patient and family/caregiver needs and revise the plan of care as necessary.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Administer medications and treatments as prescribed by the physician.
  • Counsel the patient and family in meeting nursing and related needs.
  • Provide health care instructions to the patient as appropriate per assessment and plan.
  • Maintain accurate and relevant clinical notes regarding patients' condition and care given.
  • Communicate with the physician regarding the patient's needs and report changes in condition.
  • Coordinate care plans with community health-related persons.
  • Teach the patient and family/caregiver self-care techniques as appropriate.
  • Provide and maintain a safe environment for the patient.
  • Participate in on-call duties as defined by the on-call policy.
  • Ensure arrangements for equipment and other necessary items and services are available.

Requirements

  • Current licensure as a Registered Nurse (RN) in the state.
  • CPR certification.
  • Graduate of an accredited school of nursing.
  • One to two years of recent acute care experience in an institutional setting.
  • One to two years of recent experience in home care.
  • Associate's degree required; Bachelor's degree preferred.
  • One year of hospice care experience preferred.

Nice-to-haves
