About the position

The Admissions Nurse at Bethesda Health is responsible for overseeing the admission process for patients, ensuring a smooth transition into care. This role involves assessing, planning, implementing, and evaluating patient care plans, as well as coordinating communication among various stakeholders involved in the admission process. The nurse will also be responsible for completing necessary documentation and ensuring compliance with agency policies and regulatory requirements.

Responsibilities

  • Complete the admission process including assessing, planning, implementing, and evaluating patient care plans.
  • Welcome patients and assist with the move-in process.
  • Deliver Welcome Packet, answer questions, and initiate the C.A.R.E.S. process.
  • Coordinate communication with family, nursing home staff, social workers, and the team regarding the admission process.
  • Complete initial plan of care in EMR and conduct head-to-toe assessments.
  • Ensure all appropriate treatments and orders are in place and maintain a weekly wound log.
  • Initiate all necessary admission paperwork including initial plan of care, medication list, and physician orders.
  • Verify orders with the physician and document patient's current immunization status.
  • Offer, administer, and document immunizations as appropriate.
  • Demonstrate familiarity with agency policies and regulatory requirements.

Requirements

  • Current RN License in good standing in the state of practice.
  • Graduate of an accredited school of nursing.
  • Minimum of two years of varied work experience as a professional nurse, with at least one year in a post-acute setting.
  • Bachelor's and/or Master's degree preferred.
  • Demonstrates skill in administrative performance including expertise in written and oral communications.
  • Ability to assess and respond to the needs of patients and families in varied settings.
  • Demonstrates skill in nursing practice and critical thinking.
  • Ability to cope with family emotional stress and tolerance of individual lifestyles.
  • Ability to lift, transfer, and/or move items or patients weighing 10 - 200+ lbs.
