About the position

The position involves taking primary responsibility for the admission process of patients, which includes assessing, planning, implementing, and evaluating their care plans. The role requires welcoming patients, assisting with their move-in process, and coordinating communication among family members, nursing home staff, and social workers. The nurse will also complete necessary documentation and assessments in the electronic medical record (EMR).

Responsibilities

  • Complete the admission process for patients, including assessment and planning.
  • Welcome patients and assist with the move-in process.
  • Deliver Welcome Packet and answer patient questions.
  • Coordinate communication with family, nursing home staff, and social workers regarding the admission process.
  • Complete initial plan of care in EMR.
  • Conduct head-to-toe assessments and ensure appropriate treatments are in place.
  • Add any wounds to the weekly wound log.
  • Initiate appropriate preventative measures such as skin and fall risk assessments.
  • Complete all necessary admission paperwork including initial plan of care and medication lists.
  • Verify physician orders and obtain necessary signatures.
  • Document patient's current immunization status and offer/administer immunizations as appropriate.
  • Demonstrate familiarity with agency policies and regulatory requirements.

Requirements

  • Graduate of an accredited school of nursing.
  • Current nursing license in good standing in the state of employment.
  • Minimum of two years of varied work experience as a professional nurse, with at least one year in a post-acute setting.
  • Demonstrates skill in administrative performance and effective communication.
  • Ability to assess and respond to the needs of patients and families in varied settings.
  • Demonstrates skill in nursing practice and critical thinking.
  • Ability to cope with family emotional stress and tolerance of individual lifestyles.
  • Ability to lift, transfer, and/or move items or patients weighing 10 - 200+ lbs using proper techniques.

Nice-to-haves
