About the position

The Adolescent and Young Adult (AYA) Social Worker Navigator at Cleveland Clinic is a newly created role focused on supporting oncology patients aged 15-39 years. This position involves investigating, assessing, and planning interventions to help patients and their families cope with various psychosocial challenges. The role emphasizes independent work, strong therapy skills, and the ability to adapt in a flexible environment, all while contributing to the mission of caring for life.

Responsibilities

  • Interviews patients, assesses priorities, and documents case activity.
  • Consults with team managers, statutory and voluntary agencies, and patient's relatives to ensure compliance with the patient's treatment plan.
  • Assists clients/families with obtaining community assistance by referral to proper resources.
  • Develops and maintains working relationships with community health, welfare, and social agencies to assist clients with their needs.
  • Conducts clinical services such as intakes, assessments, and psychotherapy at the CSW-I or LCSW level.
  • Documents patient care interactions within an EMR template for program oversight and growth.
  • Develops psychosocial programming specific to the AYA population, including support groups and other therapeutic interventions.
  • Utilizes specialized clinical knowledge in psychotherapy, clinical counseling, diagnostic interviews, assessments, and treatment plans.

Requirements

  • Master's degree in Social Work.
  • State Licensed Social Worker (LSW), Clinical Social Work Intern (CSW-I), Licensed Independent Social Worker (LISW), or Licensed Clinical Social Worker (LCSW) license as required by the state.
  • Two years of post-graduate employment with clinical supervision (for LISW/LCSW level).
  • Experience in hospital care or closely related field as an employee, graduate, or undergraduate intern (for LSW/CSW-I level).
  • Oncology experience is preferred.
  • Ability to utilize electronic medical record templates and computer software such as SmartForms, electronic dashboards, and spreadsheets.

  • Excellent written and verbal communication skills.
