About the position
The Adult Behavioral Health Home Coordinator (Case Manager) is responsible for assisting clients with long-term psychiatric disabilities in developing the skills and resources necessary for successful community integration. This role emphasizes holistic care across eight dimensions of wellness, ensuring clients receive comprehensive support tailored to their individual needs.
Responsibilities
- Establish compassionate therapeutic relationships with clients and prospective clients.
- Perform collaborative case management functions, linking, coordinating, and managing the delivery of services to clients.
- Provide respectful, supportive counseling to clients, assisting them in selecting and weighing alternatives and making decisions.
- Coordinate clients' physical and mental health needs.
Requirements
- A minimum of an associate degree in a Human Services related field is required.
- MHRT-C certification or MHRT-C Provisional certification is required.
- Personal vehicle use is required.
