About the position

The Adult Case Manager (ACM) plays a crucial role in providing tailored services to adults with behavioral health diagnoses who struggle with independent functioning. This position involves developing individualized service plans and ensuring that clients receive the necessary benefits and support through the Managed Care System. The ACM will work within a structured office schedule while also being required to travel within a designated area to meet clients' needs.

Responsibilities

• Develop client-centered individual service plans for adults with behavioral health diagnoses.
• Ensure services are approved through the Administrative Service Organization (ASO) Managed Care System.
• Maintain current benefit and financial information for clients with cognitive disabilities.
• Travel within the designated area to meet clients.
• Work a regular office schedule, Monday to Friday, between 6am and 5pm.

Requirements

• Proof of vehicle insurance at $50K/$100K and a valid Maine driver's license.
• Must be 18 years of age or older.
• High school diploma or equivalent.
• MHRT/C or MHRT/CP certification required.
• Intermediate level computer experience.
