About the position

The Adult Case Manager at the Brockton Multi Service Center (BMSC) is responsible for providing comprehensive assessment, outreach, support, service coordination, and advocacy for DMH authorized adults. The role emphasizes recovery-oriented practices, trauma-informed care, and cultural humility, focusing on individual strengths and choices. The case manager will work collaboratively with clients and their support systems to develop and implement individualized service plans, monitor treatment progress, and facilitate transitions to community-based services.

Responsibilities

  • Provide assessment, outreach, support, and service coordination to DMH authorized adults.
  • Conduct comprehensive assessments and screenings to determine individual service needs.
  • Develop, implement, and monitor individual service plans (ISPs) in collaboration with clients and service providers.
  • Assist individuals in securing necessary services and supports for recovery.
  • Engage in direct communication with clients in various settings, including inpatient care and community environments.
  • Facilitate discharge planning and service coordination activities to support community transitions.
  • Maintain accurate documentation according to DMH regulations and EHR business rules.
  • Participate in utilization management reviews to monitor resource use and service effectiveness.
  • Respond to and manage crises, providing support through face-to-face or telehealth contact as needed.
  • Demonstrate cultural humility and sensitivity in interactions with clients.

Requirements

  • At least three years of full-time or equivalent part-time professional experience in human services or social work.
  • A Bachelor's degree or higher in a related field may substitute for two years of required experience.
  • Proficiency in Microsoft Office applications including Outlook, Word, and Excel.
  • Ability to actively listen and demonstrate sensitivity and respect for individuals.

Nice-to-haves

  • Bilingual or multilingual proficiency in Cape Verdean dialects.
