About the position
The Adult Case Manager at Swanson Center is responsible for providing comprehensive mental health services to clients. This role involves conducting client intakes, developing treatment plans, and ensuring clients receive necessary follow-up care. The Case Manager will work both in the office and in the community, visiting clients at their homes or other facilities, and will be part of a team dedicated to improving the lives of individuals with mental health needs.
Responsibilities
- Conduct client intakes and assessments.
- Participate in treatment and discharge planning.
- Execute treatment plans and provide individual and group services.
- Ensure clients receive necessary follow-up care and assistance.
- Visit clients at other facilities or in their homes.
- Perform standard case management tasks such as linkage, monitoring, and follow-up.
- Advocate for clients to improve their general well-being.
- Assist with the preparation of commitment materials and court referral proceedings.
- Maintain accountability for assigned clients' progress and continuity of care.
- Submit all required paper and electronic records of client care and staff activity in a timely manner.
- Attend and participate in designated staff meetings.
- Develop and maintain linkages with other caregivers and the state hospital system.
Requirements
- Bachelor's degree in Social Work, Psychology, Addictionology, or a health-related field.
- At least two years of post-degree experience working in behavioral health.
- Valid driver's license and a good driving record.
- Registered four-wheeled vehicle.
