About the position

Johnson County Mental Health is seeking a qualified candidate to join our Adult Services team as a Case Manager. This role is pivotal in providing support to adults experiencing severe mental illness, and it requires a compassionate and dedicated individual who is committed to making a difference in the lives of our community members. As a Case Manager, you will work as part of a strengths-based case management team, delivering community-based services tailored to the unique needs of Johnson County residents. The position requires some evening availability, and the schedule will vary based on client needs, ensuring that we provide the most effective support possible. In this role, you will conduct thorough assessments of individuals' backgrounds and circumstances to identify their needs and determine service eligibility. You will interpret and communicate relevant information to individuals, families, and other stakeholders, ensuring that everyone involved understands the services available. Developing comprehensive plans to address the needs of individuals across various focus areas is a key responsibility. You will network with other community agencies to research and coordinate a variety of resources and services, whether for short-term intensive support or long-term individual needs. Encouraging the establishment and maintenance of support systems is essential, as is consulting and collaborating with other providers in the development, implementation, and review of these plans. As a Case Manager, you will provide a variety of services for a designated caseload of individuals with multi-faceted and ongoing needs. It is crucial to ensure that the services provided align with individual plans. You will initiate and attend individual and group meetings, providing services in community-based settings, and may also be responsible for transporting individuals as identified in their plans. Organizing and facilitating skill development and support groups will be part of your duties, fostering a supportive environment for clients. Documentation is a vital aspect of this role. You will complete and submit records regarding the services provided, evaluate and document individuals' progress according to measurable goals, and review the quality and effectiveness of the services delivered. This position is not just about providing services; it is about making a meaningful impact in the lives of those we serve and ensuring that they have the support they need to thrive in their communities.

Responsibilities

Requirements

  • Bachelor's degree in Human Services or related field required.
  • Valid driver's license with acceptable driving record required.
  • Reference/background screening required for all positions.
