About the position

The Advanced Practice Nurse (APN) III Certified Nurse Practitioner (NP) at the Department of Veterans Affairs provides comprehensive healthcare services to veterans, focusing on quality and timely care. This role emphasizes health promotion, disease prevention, and the development of personalized health plans within a Whole Health System of care. The APN operates independently, managing a panel of patients with both acute and chronic conditions, and collaborates with an interdisciplinary team to enhance patient outcomes.

Responsibilities

  • Independently manage a panel of patients with acute and chronic health conditions.
  • Develop and implement individualized health plans for optimal population health and disease management.
  • Demonstrate leadership and collaborate with the interdisciplinary team.
  • Order and interpret labs, tests, and diagnostic studies.
  • Prescribe pharmacologic and non-pharmacologic interventions.
  • Initiate referrals and consults to specialty services.
  • Provide education to patients and their families.
  • Assess and adjust the plan of care as needed to achieve optimal patient outcomes.

Requirements

  • A post-master's certificate or master's or doctoral degree as a Nurse Practitioner.
  • Active, current, full, and unrestricted APN license to practice as an NP in a state, territory, or commonwealth of the United States.
  • Full and current board certification in an area of clinical specialty from a recognized certifying body.
  • One year of specialized APN experience equivalent to the Nurse II grade level.
