About the position
The Advanced Practice Provider (APP) plays a crucial role in the interdisciplinary healthcare team, delivering high-quality and cost-effective care to patients. This position involves collaboration with a supervising physician and adherence to organizational policies and standards. The APP is responsible for patient assessments, treatment planning, education, leadership, and quality improvement initiatives, reflecting the organization's mission and values.
Responsibilities
- Obtain relevant health and medical history and perform thorough physical assessments.
- Develop and implement treatment plans, including prescribing evidence-based pharmacological agents.
- Conduct preventive health screenings based on patient needs and maintain comprehensive records of patient care.
- Educate clients and families regarding medical conditions, treatments, and follow-up care.
- Provide instruction and training to physicians, nursing staff, and students.
- Engage in continuous quality improvement and lead interdisciplinary teams to enhance patient care delivery.
- Participate in community outreach programs and develop initiatives to improve healthcare access.
Requirements
- Completion of an accredited graduate-level Advanced Practice Provider (APRN or PA) program.
- Licensed as an Advanced Practice Provider in the state of North Carolina.
- Board certification if required by the Department.
- DEA certification if applicable.
