About the position

The Advanced Practice Provider (APP) plays a crucial role in the interdisciplinary healthcare team, providing high-quality, cost-effective care to patients. This position involves obtaining medical histories, performing assessments, developing treatment plans, and collaborating with supervising physicians. The APP is expected to reflect the organization's mission and values while adhering to relevant policies and standards.

Responsibilities

  • Obtain relevant health and medical history and perform thorough physical assessments.
  • Develop and implement treatment plans, including prescribing evidence-based pharmacological agents.
  • Conduct preventive health screenings based on patient needs and maintain comprehensive records of patient care.
  • Educate clients and families regarding medical conditions, treatments, and follow-ups.
  • Lead and participate in community outreach programs and systems improvements.
  • Engage in continuous quality improvement and consult with management on clinical issues.

Requirements

  • Completion of an accredited graduate-level Advanced Practice Provider (APRN or PA) program.
  • Licensed as an Advanced Practice Provider in the state of North Carolina.
  • Board certification if required by the Department.
  • Demonstrated clinical competency and effective leadership skills.
  • Strong interpersonal, written, and verbal communication skills.
