About the position

Signature Healthcare at Home is seeking a dedicated and compassionate Registered Nurse (RN) for a full-time after-hours on-call position. This role is crucial for providing home health acute care to patients outside of regular business hours. The RN will be responsible for responding to triage nursing calls, completing patient visits, and addressing any follow-up needs for patients and caregivers. The position requires a commitment to delivering high-quality care with a focus on palliative and rehabilitative services, ensuring that the holistic needs of patients and their families are met. The RN will work autonomously, utilizing the nursing process as defined by the state Nurse Practice Act. Responsibilities include assessing and evaluating patient status, collaborating with physicians and hospice medical directors, administering medications and treatments, and providing health care instructions to patients and families. Effective communication is essential, as the RN will document patient services, report changes in patient conditions, and coordinate care with community health providers. The role also involves teaching self-care techniques and supporting patients in achieving optimal comfort and independence. This position offers flexible work schedules, including PRN, part-time, or full-time options, with 10-hour shifts available. Responsibilities

  • Serve as the field contact for Triage nursing and respond to calls during shifts.
  • Complete patient visits and follow-up for patient/caregiver needs after business hours.
  • Plan, organize, and direct Hospice and/or Home Health care as needed.
  • Provide professional nursing care utilizing all elements of the nursing process.
  • Assess and evaluate patient status and collaborate with physicians to revise care plans as needed.
  • Administer medications and treatments as prescribed by the physician.
  • Involve patients and families in addressing needs and achieving related goals.
  • Document patient services and status timely and accurately according to professional standards.
  • Communicate with physicians regarding patient needs and changes in condition.
  • Collaborate with the on-call hospice provider to adjust patient care levels as necessary.
  • Teach self-care techniques to patients and families and provide health counseling as appropriate.
  • Attend required staff meetings and interdisciplinary group meetings.

Requirements

  • Graduate of an accredited nursing school.
  • One to two years of recent acute care experience in an institutional setting.
  • One to two years of recent experience in hospice or home health care.
  • Currently licensed as a Registered Nurse in the state of practice.
  • Possess and maintain CPR certification.
  • Effective written and oral communication skills with all levels of staff and patients.
  • Exceptional interpersonal skills, both over the phone and in person.
  • Strong observation, verbal and written communication skills, and problem-solving abilities.
  • Self-directed and able to work with minimal supervision.
  • Capable of prolonged walking and lifting, positioning, and transferring patients.
  • Ability to lift supplies and equipment as needed.
  • Capable of reaching, stooping, bending, kneeling, and crouching as required for patient care.
  • Possess visual acuity and hearing to perform required nursing skills.
  • Must have a valid driver's license and an insured automobile.
