About the position

The Agency Admissions Liaison plays a crucial role in facilitating the transition of clients into the Aware Recovery Care program. This position involves collaborating with Admissions and Agency leadership to ensure that clients understand the program's expectations and eligibility criteria before they meet their care teams. A solid understanding of the treatment philosophy and admission processes is essential, along with knowledge of substance abuse and dual diagnosis.

Responsibilities

  • Communicates and collaborates with central Admissions and Agency Leadership to ensure smooth client transitions into care.
  • Scans, uploads, organizes, saves, and stores confidential documentation within the EHR.
  • Involvement in collaborative treatment team planning and coordination of treatment goals based on client needs.
  • Sets up client meet & greet with the Intake Coordination department.
  • Explains the program to patients and ensures comprehension of informed consent.
  • Explains necessary Admissions documentation to clients and obtains verbal authorization via telehealth.
  • Trained in the Aware Recovery Care admission and referral process.
  • Proficient in CRM and EHR software operation.
  • Verifies contacts, clients, providers, and referrals daily through SALESFORCE and ensures compatibility with EHR information.
  • Communicates the importance of treatment compliance and available resources for co-morbidities and dual diagnosis.
  • Explains patients' rights and responsibilities and learns all program options available to patients.

Requirements

  • Bachelor's Degree preferred.
  • 1 year of experience in addiction counseling or related field.
  • Proficient in electronic health records and customer resource management software.
  • Strong written and verbal communication skills.
  • Basic understanding of Substance and Alcohol Use Disorders.
