About the position
The Aging Case Aide 2 position at the Carbon County Area Agency on Aging is designed for motivated individuals who are passionate about supporting elderly community members. This role focuses on promoting and improving care for older adults, ensuring they can live independently and with dignity. The aide will serve as a primary contact for cases involving abuse or neglect and will assist clients in accessing various benefits and services.
Responsibilities
- Promote, protect, and improve a continuum of care for older adults.
- Serve as the main point of contact for cases involving abuse, exploitation, abandonment, or self-neglect.
- Prepare thorough Reports of Need to ensure adherence to protocols.
- Provide information and referral services to clients.
- Assist clients in applying for benefits such as PACE/PACE Net, Medicare, and LIHEAP.
- Inform consumers about available services, programs, and community resources.
- Refer clients to other agencies or service providers as needed.
- Deliver Person Centered Counseling to clients.
Requirements
- One year as an Aging Case Aide 1; or
- Two years of public contact work, including one year in a human services program; or
- Any equivalent combination of experience and training.
- Must meet PA residency requirement.
