About the position

The Aging Care Manager 2 position at the Luzerne County Area Agency on Aging focuses on providing essential support to older adults through telephone intake and case management. The role involves handling calls, gathering information, and prioritizing cases to ensure that clients receive the necessary services and support. The Aging Care Manager will also assist with various forms and reports, contributing to the overall mission of enhancing the quality of life for mature adults in the community.

Responsibilities

  • Handle telephone intake and obtain information from individuals requesting services.
  • Prioritize and assign cases based on urgency and need.
  • Conduct interviews with consumers who come into the office for information or services.
  • Maintain knowledge of community support services available for older and disabled adults.
  • Assist walk-in consumers with the completion of forms such as PACE, Tax Rebate, Rent Rebate, and Transportation service applications.
  • Complete monthly statistic reports and participate in monthly Unit meetings.

Requirements

  • Six months as an Aging Care Manager 1 or a County Caseworker 1.
  • Successful completion of the County Social Casework Intern program.
  • Six months of professional human services or nursing experience and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field.
  • An equivalent combination of experience and training.

Nice-to-haves
