About the position
The Aging Case Aide 2 position at Carbon County Area Agency on Aging is designed for motivated individuals who are passionate about supporting the elderly community. This role focuses on promoting and improving the quality of life for older adults, ensuring they can live independently and with dignity. The position involves direct interaction with vulnerable populations, addressing issues such as abuse and neglect, and providing essential information and referral services to help clients access various benefits and community resources.
Responsibilities
- Promote, protect, and improve a continuum of care for older adults.
- Serve as the main point of contact for cases involving abuse, exploitation, abandonment, or self-neglect.
- Prepare thorough Reports of Need to ensure adherence to protocols.
- Provide information and referral services to clients.
- Assist clients in applying for benefits such as PACE/PACE Net, Medicare, and LIHEAP.
- Inform consumers about available services, programs, and community resources.
- Refer clients to other agencies or service providers as needed.
- Deliver Person Centered Counseling to clients as the PA-MEDI Coordinator.
Requirements
- One year as an Aging Case Aide 1; or
- Two years of public contact work, including one year in a human services program; or
- Any equivalent combination of experience and training.
- Must meet the PA residency requirement.
