About the position

The Alumni Case Manager Coordinator at House of Hope Inc. plays a crucial role in supporting clients on their journey to recovery and independence. This position involves assessing client needs, facilitating group sessions, and collaborating with clients to create personalized recovery plans. The coordinator will also network with various service providers to ensure clients have access to necessary resources.

Responsibilities

  • Assess clients' needs and link to appropriate resources.
  • Develop and facilitate engaging group sessions that focus on relapse prevention strategies and essential life skills.
  • Collaborate with clients to develop individualized recovery plans, goals, and strategies for success.
  • Assist clients in obtaining meaningful vocational and educational experiences.
  • Network with agency and outside service providers, ensuring client access to appropriate resources.
  • Perform additional tasks and duties as assigned.

Requirements

  • Minimum of an Associate's degree in social work or psychology; Bachelor's Degree preferred.
  • Two (2) years' experience working in the field of addictions and/or mental health.
  • Knowledge of the recovery process and the ability to facilitate recovery.
  • Knowledge and skills to teach and engage in basic problem-solving strategies.
  • Ability to work with a diverse population.
  • Effective communication skills.
  • Must be an independent thinker.
  • Computer literate.
