The Ambulatory Care Nurse (ACN) at WakeMed is a Registered Nurse responsible for delivering comprehensive nursing care management services to designated patient populations. This role emphasizes a collaborative approach, where the ACN anticipates, integrates, and coordinates the patient's plan of care to ensure they can remain in a community setting. The ACN works closely with physicians to identify at-risk patient populations that require specialized care management services. In this capacity, the nurse acts as a patient and family advocate, serving as a vital communication link between the physician, patient, family, and the interdisciplinary healthcare team, as well as connecting them with community resources. The Ambulatory Care Nurse is expected to understand the organizational needs and actively support the mission, values, and management of patient care services. This includes adhering to the ANA Scope and Standards of Nursing Practice, the North Carolina Board of Nursing Practice Act, and WakeMed's Division of Nursing Professional Practice model. The ACN plays a crucial role in promoting WakeMed's Patient & Family Centered Care (PFCC) model, ensuring that care is tailored to meet the unique needs of each patient and their family. This position is integral to enhancing the overall health and well-being of the community served by WakeMed, which has been a leading provider of health services in Wake County since 1961.
