About the position

The Ambulatory Clinic RN Care Coordinator for the Gender Diversity Clinic is a licensed registered nurse responsible for providing ethical, evidence-based, and compassionate nursing care. This role focuses on patient safety and quality of care by addressing patient needs, conducting health education, advocating for patients, coordinating healthcare services, and evaluating patient outcomes. The RN Care Coordinator supports physicians by facilitating communication with patients and ensuring understanding of care plans, while managing patient flow and direct care in a clinical setting.

Responsibilities

  • Manage patient flow, triage, and direct patient care.
  • Act as a liaison between front desk staff, nursing staff, physicians, and clinic manager.
  • Develop expected patient outcomes and set patient goals in collaboration with providers.
  • Provide nursing support to optimize patient outcomes and improve quality of life.
  • Support patient self-management of diseases.
  • Coordinate continuity of care following hospital admissions, discharges, and ER visits.
  • Manage high-risk patient care, including those with multiple co-morbidities.
  • Develop relationships with patients and assess their healthcare needs.
  • Educate patients and ancillary personnel about healthcare needs and safety issues.
  • Utilize electronic tools like EMR for documentation and patient records management.
  • Observe, record, and report patient conditions and reactions to treatments.
  • Respond to patient treatment-related phone calls and provide instructions regarding medications and treatments.
  • Maintain patient confidentiality and compliance with practice standards.

Requirements

  • High School Graduate
  • Bachelor Degree in Nursing
  • 2 or more years of RN experience
  • Basic Life Support Heartsaver (BLS HS) certification (Non-Clinical) from American Heart Association (AHA) or obtained within 14 days
  • Licensed Registered Nurse (LRN) - Multi-State from State Board of Nursing

Nice-to-haves

  • Previous medical office experience
  • Current active multi-state Registered Nurse (RN) license in good standing
  • Proof of application for a multi-state RN license if only holding a single state license
