About the position

The Ambulatory Heart Failure Case Manager is responsible for providing expert assessment, diagnosis, and treatment for individuals with complex health issues related to heart failure. This role emphasizes care coordination, wellness maintenance, and comfort provision, ensuring high-quality patient care and satisfaction. The position requires independent decision-making and effective communication with clients and families to achieve positive health outcomes.

Responsibilities

  • Demonstrate expertise in assessing and diagnosing health problems related to heart failure.
  • Provide treatment and care coordination for patients with complex health responses.
  • Organize and plan work with input from clients and families to achieve specific outcomes.
  • Make appropriate decisions and initiate interventions within professional practice boundaries.
  • Determine the appropriate place of contact/intervention for clients to ensure satisfaction and positive outcomes.

Requirements

  • Current RN licensure in Virginia or eligible compact state.
  • AHA BLS HCP Certification or equivalent.
  • Baccalaureate Degree in Nursing from an accredited School of Nursing.
  • Three years of nursing experience, including two years in a specialty area.

Nice-to-haves

  • Two years of case management/care management experience.
  • Certification in a specialty area as appropriate.
