About the position

The Case Management Coordinator at CVS Health is responsible for assessing, planning, implementing, and coordinating case management activities to evaluate and facilitate the overall wellness of members. This role requires a self-motivated and detail-oriented individual who can manage a caseload of supportive and medically complex members, ensuring they receive appropriate services and support. The position involves significant travel within Miami Dade county for face-to-face member interactions, and it operates within a Monday to Friday schedule, promoting autonomy and flexibility in care coordination.

Responsibilities

• Telephonically and/or face-to-face assess members' medical needs.
• Plan and implement case management activities to enhance member wellness.
• Manage a caseload that includes supportive and medically complex members.
• Develop proactive courses of action to address member issues.
• Determine appropriate services and supports based on members' health needs.
• Coordinate with primary care providers and skilled providers for member care.
• Review medications and provide condition management information.
• Connect members with community resources and supports.

Requirements

• Bi-lingual in English and Spanish.
• 1+ year of experience in behavioral health or long-term care setting.
• Ability to travel 50-75% of the time to meet members face-to-face.
