About the position

Responsibilities

• Assessing members' medical needs telephonically and face-to-face.
• Planning and implementing case management activities.
• Coordinating care with members to facilitate overall wellness.
• Managing a caseload of supportive and medically complex members.
• Determining appropriate services and supports based on members' health needs.
• Conducting medication reviews and coordinating with primary care providers and skilled providers.
• Utilizing community resources and supports to enhance member wellness.

Requirements

• Bilingual in English and Spanish.
• 1+ year of experience in behavioral health or long-term care setting.
• Ability to travel 50-75% of the time to meet members face-to-face.
