About the position

The Analyst, Case Management Specialist at CVS Health is responsible for managing the care of members enrolled in the Dual Special Needs Plan, focusing on those with limited health conditions. This role involves collaborating with a Care Team to enhance health outcomes, address social determinants of health, and connect members with community resources. The Care Manager will assess member needs, maintain care plans, and participate in interdisciplinary team meetings to ensure comprehensive care delivery.

Responsibilities

• Interact with low stratification members via phone calls to coordinate care and update assessments and care plans.
• Create care plans based on assessments and claims data for members to follow.
• Participate in Interdisciplinary Team meetings to discuss members' healthcare needs and barriers to care.
• Identify and link members with health plan benefits and community resources.
• Perform administrative work to maintain skills needed for job duties.

Requirements

• 3+ years' experience in a health-related field.
• 2+ years of customer service experience.
• 2+ years of experience with claims.

Nice-to-haves

• Master's degree preferred.
• CRC, CDMS, CRRN, COHN, or CCM certification.
• Medicare and Medicaid experience.
• Managed care experience.
• Experience working with geriatric special needs, behavioral health, and disabled populations.
• Knowledge of assessment, screenings, and care planning.
• Bilingual (English/Spanish; English/Creole).
